Scope and Content: Item is a chart used to track the blood, urine, diet in grams, and dietary prescriptions in grams. It is 1 page and filled out by hand. Chart shows that by Sept. 3rd, Hughes gained 9 pounds from the time of her first insulin injection on Aug. 17th.
